Child’s passport

New passport (in the first paragraph)

If the applicant is under age 16, the applicant should to apply child's passport. Please note: U.S. Passport issued to applicant under 16 is valid for 5 years only, and the child's passport cannot be renewed when it is expire.

Necessary documents requirement

Note

Minors under age of 14:

  • Must appear in person.
  • Have a parent or guardian present photo identification if the minor does not have identification of his or her own
  • Must be accompanied by BOTH PARENTS or legal guardians with acceptable proof of ID. If only one parent appears the other parent must sign and notarize a Statement of Consent ( Form DS-3053 ).

Minors ages 14 to 17:

  • Must appear in person.
  • Parental consent may be requested.
  • If the child does not have identification of his/her own, a parent needs to accompany your child, present identification and co-sign the application.

NOTE: Parents/guardians will need to present evidence of their relationship to the child. Such evidence includes:

  • Minor's certified U.S. birth certificate with both parents' names or
  • Minor's certified Foreign Birth Certificate with both parents' names or
  • Certification of Birth Abroad (Form DS-1350) with parents' names listed; or
  • Adoption Decree listing adopting parents' names; or
  • Court Order Establishing Custody; or
  • Court Order Establishing Guardianship.

 

2- New passport form

               Complete and fill out our order form

3- Completed DS-11 form (application for new U.S. passport)

Kindly check the type of passport you applying for then click on the link at the bottom of this section to complete, and create your passport application (DS-11) online. After you fill out the form online, you will see at the bottom of the page this sentences “I have read and acknowledge the steps contained above “make check in the check box and you will see a button that says "Create Form", please click the "Create Form" button, and your application form (DS-11) will appear completed on your screen.

Important:

1.      Please print one copy of the completed application form (DS-11).

2.      Do not print the form on double sided pages.

3.      Do not sign this application form until you are instructed to during Authorization.

4.        If you don't have any travel plan, or your departure date is not within 14 days, please make sure do not fill in the Travel Plans section online, and leave it blank.

5.     After filling out and printing the form close the window to return to your passport instructions.
               

            4- Letter of Authorization

Download, complete, print out, date and sign the Authorization Letter. . Important: Please enclose an additional copy of the Letter of Authorization outside of the envelope sealed by the Acceptance Agent.

               5- Two identical passport photographs

Taken within the past 6 months, showing current appearance. Must be official passport-type only and approximately 2" x 2" with a clear background. The photo must be in color. Do not wear a hat or any item that covers the face. Face forward only, no side shots or profiles will be accepted. Homemade Digital Photographs are not acceptable.

 

 

 

               6-Proof of U.S. Citizenship (one of the following documents)

  • Original of  U.S. birth certificate
  • Original naturalization certificate

7- Proof of Parent's Identification

State issued driver's license (at least 6 months old) bearing a photo .

Military I. D. bearing a photo

8- Proof of Travel itinerary (optional)

You must provide one of the following items:

  • A copy of your air plane ticket or
  • A copy of your itinerary from the travel agent or airline or

9- Fees & Payment

1. $ 145 U.S. Government Fees (two separate checks or money orders):

Make one check or money order payable to "U.S. Department of State"
- for $120  

a. Write applicant's full name and date of birth on the check or money order.
b. All personal checks must show a printed address.
c. Do not cross out and rewrite any information on the check.
d. Temporary or starter check is not acceptable.

Make the second check or money order in the amount of $25 payable to the Post Office or Court House which you visit.

2. (Our fee) to fast visa. Also include payment for the return shipping option you will select.

Get Your Application Authorized

 Now you should authorize all your documents by local government officials so get your document and go to the nearest Passport Acceptance Facility  and notify the agent you are using an expediting service to "hand carry" your application

Then present all prepared required documents to the Acceptance Agent for approval. You will then sign the passport application form in the presence of the Acceptance Agent who will witness your signature and select the all your documents to place in an envelope.

  • Application form (DS-11)
  • Two passport photos
  • Proof of US citizenship
  • Proof of Travel itinerary (optional)
  • Letter of authorization  
  • The $120 check to "U.S. Dept. of State"

Important: Do not open this sealed envelope or the application will be invalid. And Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of Executed Applications".
