Passport Name Change Passport Name change (in the first paragraph) If your legal name changed from the name in your current passport and your passport was issued within one year ago. So you can choose the passport Name change service it will be the right service for you. But If your passport was issued more than one year ago please apply for U.S. passport renewal. Necessary documents requirement 1- New passport form Complete and fill out our order form 2- Completed (DS-5504) form (application for U.S. passport Name Change) Kindly check the type of passport you applying for then click on the link at the bottom of this section to complete, and create your passport application (DS-5504) online. After you fill out the form online, you will see at the bottom of the page this sentences “I have read and acknowledge the steps contained above “make check in the check box and you will see a button that says "Create Form", please click the "Create Form" button, and your application form (DS-5504) will appear completed on your screen. Important: 1. Please print one copy of the completed application form (DS-5504). 2. Do not print the form on double sided pages. 3. You must sign and Date the completed application form (DS-5504). Date your application on the day we will receive it, not the day you sign it. Ex) if you are shipping application to us on Friday Jan 2, date it for Monday Jan 5. 4. printing the form close the window to return to your passport instructions. 3- Two identical passport photographs Taken within the past 6 months, showing current appearance. Must be official passport-type only and approximately 2" x 2" with a clear background. The photo must be in color. Do not wear a hat or any item that covers the face. Face forward only, no side shots or profiles will be accepted. Homemade Digital Photographs are not acceptable 4- Letter of Authorization Download, complete, print out, date and sign the Authorization Letter. . Important: Please enclose an additional copy of the Letter of Authorization outside of the envelope sealed by the Acceptance Agent. 5-Valid US passport Please make sure that the expiration date is more than 12 months away otherwise, you need to apply for passport renewal.

Get Your Application Authorized Now you should authorize all your documents by local government officials so get your document and go to the nearest Passport Acceptance Facility and notify the agent you are using an expediting service to "hand carry" your application Then present all prepared required documents to the Acceptance Agent for approval. You will then sign the passport application form in the presence of the Acceptance Agent who will witness your signature and select the all your documents to place in an envelope.
Important: Do not open this sealed envelope or the application will be invalid. And Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of Executed Applications".
